Providing healthcare services to the homeless community can be uniquely challenging, as these individuals often move frequently and have multiple chronic conditions. Well-designed, targeted care coordination can substantially improve health outcomes for this population. New innovations developed to support the homeless, including electronic health records that store patient information in the cloud and mobile care in accessible locations such as grocery stores have the potential to improve mainstream medicine and serve as a foundation to improve access to quality care for all.
